Monroe County Health Department now has online form for positive Covid-19 cases

Monroe County is experiencing increasing levels of Covid-19 and significant spread in our community. We have seen a significant increase in the number of daily new cases in past weeks, and the number of cases continue to rise. 

If cases continue to rise, there may be times where Monroe County Health Department case investigation and contact-tracing efforts will become strained or at capacity and may not be able to contact positive cases within 24 hours. 

To help ease disease investigation and contact tracing efforts or for those wanting to save time, we are offering the option of online case interviews. To access the online form, go to healthymonroecowi.org/positive and click on the “COVID-19 Interview Form” link. This will take positive cases to an online interview form. Once the form is submitted, individuals will receive an email detailing instructions for isolation and contacting close contacts as well as links to request isolation and quarantine letters. 

All information gathered is in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and is kept confidential and accessible to health department staff only. Those who have already received a phone call from Monroe County Health Department staff do not need to complete the online form.
